Agree that maintaining the context and flow is critical especially when it comes to reviewing your notes! I wouldn't want to have floating bullet points either and not understand why I jotted them down.
So on my end, I ensure three things: (1) For the minutes bank, the key information should follow a proper outline & organized structure, (2) For the remaining banks, I write full, coherent sentences that mention the topic/discussion point it's related to (ex. What's the rationale behind strategy X for Project ABC?), (3) Each meeting should have its own set of the four mind banks.
